Waiver and Conditions of Enrollment

I/we, the parent or legal guardians, undertake and agree to indemnify and hold completely blameless Emmanuel Baptist Church of Barrie (Emmanuel), its pastors, board of elders, staff and/or volunteers from and against any loss, damage or injury suffered by the participant as a result of being part of the activities of Emmanuel, as well as any medical treatment authorized by the supervising individuals representing the church. I/we authorize the Emmanuel church staff or ministry volunteer staff to sign consent for medical treatment, and to authorize any physician or hospital to provide medical assessment, treatment or procedures for the participant named above in the event that the parent/guardian or provided emergency contact cannot be immediately reached. This consent and authorization is effective only when participating in and/or traveling for events of Emmanuel’s ministries. I/we also grant permission for the reasonable use of pictures and/or videos of the participant taken during ministry time for promotional purposes for Emmanuel. The program director reserves the right to dismiss a participant who, in the director’s opinion, is a hazard to the safety and the rights of others, or who appears to have rejected the reasonable rules of the program. By signing my name and dating below I confirm that I have fully read, understand and agree.
