(*if you have already completed the Medical/Allergy Form for the 2022/2023 year, and there are no changes, you do not need to complete this form again.)

EMMANUEL WAIVER: 

I/we, the parent or legal guardians, undertake and agree to indemnify and hold completely blameless Emmanuel Baptist Church, its pastors, board of elders, staff and/or volunteers from and against any loss, damage or injury suffered by the participant as a result of being part of the activities of Emmanuel’s Ministries, as well as any medical treatment authorized by the supervising individuals representing the church. I/we authorize the Emmanuel church staff or ministry volunteer staff to sign consent for medical treatment, and to authorize any physician or hospital to provide medical assessment, treatment or procedures for the participant named above in the event that the parent/guardian or provided emergency contact cannot be immediately reached. This consent and authorization is effective only when participating in and/or traveling for events of Emmanuel’s Ministry. Emmanuel Ministry activities & events may include, but are not limited to indoor/outdoor sports & competitive games, offsite activities, service projects, and small group meetings. I/we understand that Emmanuel encourages ministry leaders to keep in contact with students and their families for ministry purposes. I/we also grant permission for the reasonable use of pictures and/or videos of the student taken during ministry time for promotional purposes at Emmanuel. By signing and dating below I confirm that I have fully read, understand and agree.
